Medical Surgical Skilled Nursing: Our nurses provide skilled assessments, teaching on disease and medication management, wound care, enteral feedings, injections, and IV infusions to adult and geriatric patients in the home setting. Our nurses involve the patient, family, and caregiver in the treatment plan. They routinely evaluate the patient's condition and report findings to the physician. The nurses coordinate care with other health care professionals to meet the patient's rehabilitation needs in the home.
Home Health Aides: All of our certified nursing assistants work under the direction of a Registered Nurse or therapist to provide personal care for the patients.
Maternal/Pediatric: Our Maternal/Pediatric Program is designed to provide safe, quality care for mothers, infants and children in the comfort of their home. Services include assessments, teaching, infusions, phototherapy, wound care, tube feedings and lab draws.
Diabetes Education: Our registered nurses provide assessment and teaching for newly diagnosed and long term diabetic patients. Registered nurses assist in planning the teaching programs to help patient and caregiver balance food, medication, and exercise.
Wound Care: Our nurses are specially trained to care for patients with complex wounds, ostomies and incisions.
Rehabilitation Services: Physical Therapy: Our physical therapists work to restore patients to their highest level of function by utilizing exercise programs, education, ultra-sound, and gait training.
Occupational Therapy: Our occupational therapists provide teaching and training for patients and caregivers in techniques to promote independence in activities of day to day living, such as dressing and eating.
Speech Therapy: Our speech therapists evaluate patients with hearing, speech, and swallowing disorders and design individualized plans to assist patients in reaching their highest potential.
Support Services: Medical Social Worker: Our medical social workers provide a skilled evaluation of the patient’s social and emotional needs that directly affect their illness and psychosocial status. They also provide counseling for long range planning and decision making while making every effort to identify and utilize community resources.
Dietician: Our registered dietician provides a nutritional assessment, diet plan and meal planning instructions, as well as assists the patient and caregiver in ways to maintain a special diet.
Costs: Services are covered by Medicare, Medicaid, Private insurance, and Private pay. |
